For transport, Life Link III requires a completed and signed PCS Form, a hospital Face Sheet and a copy of the Hospital Transfer Orders. Additionally, if the patient is on a court ordered psych hold, Life Link III also requires the hold paperwork for transport. If you need a copy of the PCS Form, you may download one here.
